Step #1: Applicant will fill ration card form and submit to CSC centre
Step #2: CSC centre operator will feed application which has 6 fields

Now, you have to complete the registration process in 5 steps,
Step #1: Registration Format – Download Application Form for offline submission. Fill and submit the application form to the concerned authorities with necessary documents
